Azzariti was born Jan. 30, 1949, in the Bronx, to Frank and Frances Azzariti. He attended Mount St Michael and received B.S. in mechanical engineering from Manhattan College. As the founder of Hamilton Music Productions, he performed under the name Guy Hamilton. He was also the former owner of Executive Billiards in White Plains.
Azzariti is survived by his wife, Mary; stepson, John Cacciatore; sister-in-law, Kathleen Azzariti; two nieces; seven grand nieces and nephews. He was predeceased by his parents; brother, John Azzariti.
A Mass of Christian burial will be celebrated at 10 a.m. Monday, May 12 at Our Lady of Sorrows Church. He will be laid to rest at Mt Calvary Cemetery.
